Our Story

Assistance WA was founded in Western Australia by a young Registered Nurse who saw the importance of combining professional healthcare knowledge with genuine, person-centred support.

Our experience in nursing and disability support has shown us that great care is about more than completing tasks. It is about building trust, listening to the individual and understanding what matters to them.

As a small local provider, we have chosen to take a more personal approach. We want participants and their families to feel that they are known, heard and supported, rather than simply being another name on a large provider’s list.

Professionalism You Can Trust

Horizon Assistance WA is a registered NDIS provider committed to meeting the standards expected of registered providers and delivering quality, safe and participant-focused services.

Frequently Asked Questions

What is the NDIS?

The National Disability Insurance Scheme (NDIS)provides funding and support to eligible Australians living with permanent and significant disability. The NDIS helps participants access the supports and services they need to achieve their goals, increase independence and participate in their community.

Who can access the NDIS?

Generally, you may be eligible for the NDIS if you are an Australian citizen, permanent resident or Protected Special Category Visa holder, are aged between 9 and 65 when you apply, and have a permanent and significant disability that substantially affects your ability to take part in everyday activities. There are also early intervention pathways for some people who do not meet the disability requirements.

How can I use my NDIS funding with Horizon Assistance WA?

If Horizon Assistance WA provides a service that is included in your NDIS plan, you may be able to use your funding to access our support. The way you can engage us depends on how your plan is managed —self-managed, plan-managed or NDIA-managed— and whether the support is reasonable and necessary and covered by your plan.

How do I get started with Horizon Assistance WA?

Getting started is simple. Contact our friendly team to discuss your needs, goals and the type of support you are looking for. We can discuss your NDIS plan, answer any questions you may have and determine how Horizon Assistance WA can support you.
